29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the actions listed and was not done in sequence by 29 CFR 1910.147(d)(1) through (d)(6): (1) prepare for shut down by recognizing energy hazards and its controls, (2) turn off and shut down the machines, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) affix lockout or tagout devices to each energy isolating device, (5) render safe all potentially hazardous stored or residual energy, (6) verify that isolation and de-energization of the machine or equipment have been accomplished. a) On or about January 24, 2025, the employer failed to implement energy control application steps prior to allowing employees to perform setup tasks such as, but not limited to, changing of screen-pack and breaker plate assembly of the hydraulic screen changer after the equipment sat idle for a prolonged period of time while the barrel and heating element continued to be heated and sustained high melt temperatures. Operators were exposed to an unexpected release of hazardous stored pressure, heated gases and fumes, and heated molten plastic from the extruder barrel likely to result in serious burns and other injuries. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
