29 CFR 1910.23(d)(5): The employer did not ensure that for through ladders, the steps or rungs are omitted from the extensions, and the side rails are flared to provide not less than 24 inches (61cm) and not more than 30 inches (76 cm) of clearance: 194 Cleveland St. Greenfield, MA: On or about January 24, 2025, and at times prior, employees were exposed to fall hazards when they used a 16 feet 6 inch tall through ladder that did not have flared side rails, and provided less than 24 inches of clearance when accessing the second floor of the mixing plant.
