29 CFR 1910.29(b)(2)(i): The employer did not ensure that guardrails met the requirement that midrails that are installed at a height midway between the top edge of the guardrail system and the walking-working surface: On or about December 19, 2024, it was observed that employees engaged in material handling in the shipping department of Behlen MFG. CO. located at 4025 East 23rd Street, Columbus, Nebraska were exposed to fall hazards in that employees accessed the overhead crane cab approximately 155 inches above the next lower level via a fixed stairway where two sides of the guardrail did not have a midrail of approximately 20 inches and no top rail.
