OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards. On or about November 26, 2024, and times prior thereto, at the facility in the testing area, a) Employees working within the blast area while conducting an above ground high pressure test were exposed to hazards of being struck by debris. b) Employees using a heater tool where water contamination occurred inside the heater tool during a high pressure test were exposed to hazards of being struck by debris when the heater tool ruptured.
