29 CFR 1910.119(f)(1)(i)(G):The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and did not address the startup following a turnaround, or after an emergency shutdown: a)AMTEC Corporation dba Tech Ord, a Division of AMTEC Corporation at 47600 180th Street, Clear Lake, SD 57226: On or about and at times prior to November 25, 2024, the employer did not provide in the Product Realization Operating Procedure Manufacture of PY-951 Document Number 710054 Revision N clear instructions for Start-Up Following an Emergency Shutdown for PY-951 transferred to velostat trays, as described in Steps 5.4.9 and 5.4.10, where screening is not initiated prior to the emergency shutdown. This condition exposed employees to explosion hazards.
