29 CFR 1910.147(d): The application of energy control (lockout or tagout) actions were not done in the following sequence: (1) prepare for shutdown, (2) shut down machine or equipment, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) affix lockout or tagout devices to each energy isolating device by authorized employee, (5) relieve, disconnect, restrain or otherwise render safe all stored or residual energy, (6) verify that isolation and deenergization of the machine or equipment have been accomplished: (a) On or about November 25, 2024, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of hazardous energy sources were performed prior to employees setting up the Booklet Open End machines (BOE's). Employees performed set-up work tasks on BOE machine, model number - 601, Serial number - 4637, without deenergizing each source of energy and placing a locking device on the energy isolating device for each identified energy source within the written procedure. The employer failed to implement energy control application steps as the machines were not isolated from all energy sources [per the 1910.147(d)(3) requirements]. As a result, the remaining applicable energy control elements, involving LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.
