29 CFR 1910.119(e)(3)(vii): The process hazard analysis did not address a qualitative evaluation of a range of the possible safety and health effects of failure of controls on employees in the workplace: (a) Throughout the facility; On or about November 20, 2024, the process hazard analyses, including but not limited to, PHA-PCP-583, PHA-PCP-634 , PHA-PCP-732, PHAs per buildings 2 and 10, and the PHA per GWI-SAF-4 Respiratory Protection did not sufficiently address employees' potential exposure to heavy metals including, but not limited to, lead and cadmium; to include failure of and lack of engineering controls such as ventilation, work practices, and personal protective equipment.
