29 CFR 1910.29(b)(2)(i): The employer did not ensure that guardrails met the requirement that midrails that are installed at a height midway between the top edge of the guardrail system and the walking-working surface: On or about November 20, 2024, an employee working from the raised platform attached to the 48-inch mill located in Building B was exposed to a fall hazard of approximately 4'-6" because the guardrail located opposite of the mill did not contain a midrail.
