29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about November 15, 2024, during a water line replacement at a worksite located near 354 E. 2OOTH ST., EUCLID, OH 44119, the employer did not ensure employee(s) working inside a trench, with varying approximate depths from 3.5-foot walls up to 5.9-foot walls, were protected from cave-ins by an adequately designed protection system. The soil would have been classified as; Type "B". TERRACE CONSTRUCTION COMPANY, INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 1432190, citation number 01, item number 01b, and was affirmed as a final order on 05/18/2020 with respect to a workplace located at 16935 LAKE ROAD, LAKEWOOD, OH 44107.
