29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag were not placed on each disconnecting means used to deenergize circuits and equipment on which work was to be performed: a) River Well Motor Control Center: On or about October 29, 2024, an employee was exposed to shock hazards while working inside of a motor switch control panel due to not placing a lock and tag on the primary disconnect means to de-energize the circuit supplying the switch panel. Abatement certification and documentation are required.
