29 CFR 1910.269(a)(4): Existing characteristics and conditions of electric lines and equipment that were related to the safety of the work to be performed was not determined before work on or near the lines or equipment was started. Such characteristics and conditions include, but not limited to 1910.269(a)(4)(i) through 1910.269(a)(4)(viii): a) Location-320 West Main St., Sayreville, NJ 08872- JCP&L Substation On or about 5/24/2024, contractor employee(s) conducted air quality testing within a JCP+L sub-station using hand-held multi-gas meter(s) and air sampling pumps, such as but not limited to, a Lighthouse 3016-iaq Particulate meter. Electrical lines and/or conductors housed within the sub-station were energized during air quality testing work by employee(s). Characteristics and conditions of lines and equipment within the sub-station building, including nominal voltages, were not determined and shared with the contract employer before contractor work was to be performed, exposing contractor employee(s) to electrical hazard(s). An arc flash incident occurred on 5/24/2024, in which employee(s) sustained burns to both hands and face.
