29 CFR 1910.147(c)(4)(ii)(D): Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. (a) On or about October 4, 2025, the employer did not ensure that the energy control procedures addressed all potentially hazardous energy sources for employees who were performing serving operations on equipment such as, but not limited to, the 2189 Pointer Machine. Servicing operations included, machine setup, and clearing a jam. Noted deficiencies included, but may not be limited to, the following: Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other control measures pursuant to 1910.147(c)(4)(ii)[D]. The procedure lacked specific steps to verify that isolation and de-energization was accomplished, such as but not limited to attempting to restart the equipment, dissipating hazardous heat, operating relieve valves, and otherwise rendering potentially hazardous stored or residual energy safe.
