29 CFR 1910.147(d)(4)(iii)(B): Where a tag could not be affixed directly to the energy isolating device, the tag was not located as close as safely possible to the device, in a position that would have been immediately obvious to anyone attempting to operate On or about October 10, 2024, in ARU #6/7 at Deer Park Refinery Limited located in Deer Park, TX, the host employer (DPRL) failed to affix tag(s) on the energy isolation device for 12-inch CAG header and /or flare lines in ARU#6/7.
