29 CFR 1926.1203(e)(2)(iii): Before an employee entered the space, the internal atmosphere was not tested as required and/or any employee who entered the space, or that employee's authorized representative, was not provided an opportunity to observe the pre-entry testing required by this paragraph. On or about September 12, 2024, and times prior thereto, employees conducted sediment/sludge extraction operations at the floor of the lift station workspace approximately 42 feet below ground level accessed by ladder. The workspace internal atmosphere was not tested prior to employees entering the workplace as required.
