29 CFR 1910.333(b)(2)(ii)(B):The circuits and equipment to be worked on were not disconnected from all electric energy sources: a. At the mixing plant, on or about September 4, 2024, the employer exposed employees to a 120-volt electrical hazard, in that, the employer did not ensure that all electric energy sources were disconnected during maintenance and replacement of a circuit disconnect cabinet for the IMP screw machine.
