29 CFR 1910.147(c)(5)(ii): Lockout devices and tagout devices were not singularly identified; were not the only devices used for controlling energy; were used for other purposes; and did not meet the requirements of subparagraphs (A) through (D): Employees engaged in machinery activities in the fabrication department shop were exposed to electrical shock, amputation, struck-by, caught-in and caught-between hazards in that devices other than lockout devices and tagout devices were used for controlling energy of the malfunctioning Peck, Stow & Wilcox roller machine, Model # 390-E, Serial # 3/61. It was improperly tagged out of service. The tagout device was not standardized, substantial, identifiable, but a Quality Assurance Card or a Part Rejected Card was inappropriately used as a makeshift tagout device, this is not a typical normal tagout device.
