The employer must ensure: Portable ladders used to gain access to an upper landing surface have side rails that extend at least 3 feet (0.9 m) above the upper landing surface (see Figure D-1); a) On or about September 18, 2024 at VF1, the employer did not ensure that the 8 foot A frame Werner portable stepladder being used to gain access to the 8 foot upper platen at the mold station by employees during changeovers, had side rails that extended at least 3 feet above the upper landing surface.
