29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard: On or about July 11, 2024, an employee of Georgian Gardens Healthcare LLC was removing tree branches and debris from the roof of the building located at 1 Georgian Gardens Drive in Potosi, Missouri. The employer did not provide fall protection training to employees who are or have the potential to be exposed to fall hazards. An employee was exposed to fall hazards while working on a roof.
