29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: Warehouse- On or about July 8, employees were not protected from struck by hazards where pallets of corrugated cardboard, weighing approximately 450 lbs, were stacked 4 tiers high causing the tops of lower tiers to crush and crown. The top tier pallet fell off of the stack when the employer tried to disassemble the stack because it was leaning to one side.
