29 CFR 1926.1203(b)(1): Where the workplace contained one or more permit spaces, the employer who identified, or received notice of, a permit space did not inform exposed employees by posting danger signs or by any other equally effective means of the existence and location of, and the danger posed by, each permit space. On or about July 8, 2024, at job site located at 9220 Old Santa Fe Road, in Kansas City, MO, the employer failed to ensure employees were informed of the existence, location, and dangers posed by permit spaces. The company had a 2-person laborer crew enter active sewer systems via manholes during preparation work for invert installation in a manhole for a new apartment complex.
