29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition a.) 26 East Chestnut Street, Lancaster, PA - Chestnut Street side - the metal access grate to an open ventilation shaft/pit was not replaced following maintenance activities, creating a fall hazard from a height of approximately 15 feet to a lower level, on or about June 9, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
