29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: a. On or about May 6, 2024, and at times prior, located at 1755 Hillard Rome Road, Hilliard, Ohio, 43026: Maintenance employees were exposed to struck-by hazards related to the removal of a 19,000 lbs. counterweight from a 349F excavator. The employer did not develop clear and specific procedures for the control of hazardous energy on earth moving equipment. The procedures did not contain adequate information to include the following: (1) A specific statement of the intended use of the energy control procedure was not created. [(c)(4)(ii)(A)]. (2) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (c)(4)(ii)(B)]. (3) Specific procedural steps for the placement, removal of lockout devices or tagout devices, specifically "Do not operate" tags on the equipment [(c)(4)(ii)(C)]. (4) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures [(c)(4)(ii)(D)].
