29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years. On and before March 21, 2024, with respect to an establishment located at 513 W. 9th Avenue, Oshkosh, WI 54903, the employer did not ensure that each powered industrial truck operator had been evaluated within the previous three years.
