29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole: a.) PJ Murphy Forest Products Corp., shaker area - on or about, and at times prior thereto: Employees cross over a hole in a metal platform to access another platform. The hole opening was 3-8 inches between the two platforms. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
