29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: a. On or about January 4, 2024, employees performed maintenance, setup/changeover, and/or cleaning activities on the U.S. Industrial Machinery Hydraulic Shear Model US825. The energy control procedure developed by the employer, Work Instructions-Hazardous Energy Control - Lockout/Tagout WI4.9, did not contain clear and specific instructions related to the shear being serviced to include: (1) A specific statement of the intended purpose and use of the procedure [(c)(4)(ii)(A)] (1) Specific procedural steps for shutting down the equipment, identifying and isolating the electrical, pneumatic, and hydraulic energy, and securing the energy isolating device(s) in order to control the energy in the system. [(c)(4)(ii)(B)] (2) Specific steps for the removal and/or transfer of lockout devices [(c)(4)(ii)(C)] (3) Specific steps to verify the effectiveness of the lockout measures [(c)(4)(ii)(D)] b. On or about January 11, 2024, employees performed maintenance, setup/changeover, and/or cleaning activities on the Nisshinbo HIQ-1250 CNC Punch Press. The energy control procedure developed by the employer, Work Instructions-Hazardous Energy Control - Lockout/Tagout WI4.9, did not contain clear and specific instructions related to the punch press being serviced to include: (2) A specific statement of the intended purpose and use of the procedure [(c)(4)(ii)(A)] (3) Specific procedural steps for shutting down the equipment, identifying and isolating the electrical, pneumatic, and hydraulic energy, and securing the energy isolating device(s) in order to control the energy in the system.
