29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: On or about January 8, 2024 the employer K&M Transport LLC, at jobsite located at 8702 W. State Route 18, Lot 91 Fostoria OH 44830, did not ensure that employees were instructed in the recognition and avoidance of unsafe conditions and the regulations applicable to the work environment and to control or eliminate any hazards or exposures to injury. Employees performing mobile home teardown and transportation preparation activities were exposed to struck-by and crush-by hazards while jacking/lifting a 1994 Fleetwood mobile home (S/N TNFLR26A38998RL).
