29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: (a) On or about October 23, 2023, and at times prior thereto, in the production area, where employees were exposed to electric shock hazards when they operated a DeWalt miter saw that had a frayed power cord and the saw was not taken out of service.
