29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): a) Imperial Bag & Paper Co. LLC located at 350 4th Street, Perrysburg, Ohio, 43551: On or about October 19, 2023, the employer failed to ensure employees were protected while performing work that required employees to stand on pallets of the tiered metal racking system. Employees access the elevated levels of the metal racking system, without the benefit of effective fall protective systems were exposed to falls of approximately 18- 25 feet to the concrete floor below.
