29 CFR 1910.134(m)(2)(i)(C):The employer did not establish a record of the qualitative and quantitative fit tests administered to an employee which included the specific make, model, style and size of respirator tested: a) Divine Rehabilitation and Nursing at Shane Hill located at 10731 State Route 118 in Rockford, Ohio; On or about October 5, 2023, the employer did not ensure the record of the qualitative fit tests administered to employees required to don N95 filtering facepiece respirators included the specific make, model, style, and size of respirator tested. The employer required employees to wear N95 filtering facepiece respirators to protect against the SARs-CoV-2 virus while providing care such as, but not limited to, bathing, feeding, and dressing, residents suspected or confirmed positive with COVID-19.
