29 CFR 1910.269(n)(2): For any employee to work transmission and distribution lines or equipment as deenergized, the employer did not ensure that the lines or equipment were deenergized under the provisions of paragraph (m) of this section and did not ensure proper grounding of the lines or equipment as specified in paragraphs (n)(3) through (n)(8) of this section: On or about 9/19/2023, at the Camp Lake Substation, 13303 Sullivan Road, Clermont, Florida, 34715, while replacing electrical insulators, Transmission Construction and Maintenance employees were exposed to electric shock and electrocution hazards because proper grounding of overhead powerlines was not conducted at the site. Duke Energy work methods are in place that require grounding to be completed before starting work and requires employees to maintain minimum approach distance until energized equipment is properly deenergized, tested, and grounded, while wearing all required personal protective equipment.
