29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. a.) On or about September 29, 2023, the employer did not ensure that the non factory installed attachment, the 15-16 foot probes used on the Toyota Forklift Truck Model: 50-8FCCU25 Serial #: 14352, and the Crown stand-up forklift were marked to identify the probe attachments and to show the approximate weight of the truck and attachment combination at maximum elevation with loads laterally centered.
