29 CFR 1910.178(a)(5):The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: At a job site located at 160 JD Power Trail, Jefferson City, MO; An employee encountered a fall from a work platform forklift attachment which resulted in an in-patient hospitalization. The fall hazards encountered were on or around September 22, 2023. The forklift front-end attachment used in the incident was not marked with any identifying the attachment, weight limits, or any other markings that identified possible hazards.
