29 CFR 1910.178(l)(3)(ii)(B): Operator did not ensure the load manipulation, stacking, and unstacking of the load. On or about September 12, 2023, at 3420 Plano Parkway, The Colony, Texas, employees at the jobsite operating equipment were not provided sufficient hazard recognition and avoidance training when stacking and unstacking loads on rough terrain or uneven surfaces in the following instances (a-e): a) large open holes for the spa tubs were present and exposed employees and equipment operators of the JCB 506-36, 6000-pound rough terrain forklift to fall hazards, b) uneven ground conditions with slopes with a 25-degree and 39-degree incline exposed equipment operators to tip-over hazards, c) equipment operators of the JCB 506-36, 6000-pound rough terrain forklift lifting and carrying uneven and unsecured loads to tip-over and struck-by hazards, d) equipment operators lifting loads at heights of up to 20 feet above the cab and moving the load on uneven surfaces exposed employees to tip-over and struck-by hazards,
