29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy had not been put into place. Location: 100 Emancipation Dr. Hampton Va. On or about: 17 June 2023. a) Employees performed an electrical air-switch repair for Veterans Medical Center Buildings (B148), the air switch was not effectively de-energized. Procedures were not clearly outlined to control hazardous energy.
