29 CFR 1910.212(a)(3)(ii): Points of operation on machinery were not guarded to prevent employees from having any part of their body in the danger zone during operating cycle: (a)Alkota Cleaning Systems, Inc. at 105 Broad Street, Alcester, SD 57001: On or about and at times prior to July 5, 2023, the employer did not ensure the Scotchman Brake Press: model number: 6500, serial number: 5575FFA76 in the Research and Development area, was properly guarded. This condition exposed employees to caught-in, crushing and amputation hazards associated with the point of operation.
