29 CFR 1926.1053(b)(15):Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use. Location: Worksite On or about June 22, 2023, employees were exposed to fall hazards of more than 20 feet where no competent person had inspected the extension ladder on site for visible defects, and the broken ladder was being utilized by employees to access the roof. A rung of the ladder was broken.
