29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): (a) Building F2, pallet rack blasting and paint line area; A floor hole opening between the media blaster and a platform attached to the north side of the media blaster was not covered or guarded to prevent employees from stepping and/or falling into it.
