29 CFR 1910.134(d)(1)(i): Selection of appropriate respirators was not based on the respiratory hazard(s) to which the worker was exposed and user factors that affect respirator performance and reliability 5 Skylark Road, Greenwich CT: On or about April 30, 2023, the employer did not select and have the sprayer use a supplied air respirator when exposed to Diphenylmethane Diisocyanate (MDI) in an area with no ventilation for the worker performing tasks such as (but not limited to) insulation spraying for an 8-hour shift.
