29 CFR 1910.95(m)(1): The employer did not maintain an accurate record of all employee exposure measurements required by 29 CFR 1910.95(d): On or about May 11, 2023, and at times prior thereto and thereafter, an accurate record of all employee exposure measurements was not maintained by the employer. Employees were exposed to an 8-hour time-weighted average of 85 decibels or greater exposing employees to the hazard of hearing loss.
