29 CFR 1915.74(a)(1): A gangway of not less than 20 inches walking surface of adequate strength, maintained in safe repair and safely secured was not used. If a gangway is not practical, a substantial straight ladder, extending at least 36 inches above the upper landing surface and adequately secured against shifting or slipping was not provided. When conditions are such that neither of gangway nor a straight ladder can be used, a Jacob's ladder meeting the requirements of (d)(1) and (2) were not used. M/V Catalina Express: On and/or before April 11, 2023, employee(s) were installing life rafts on the top deck of the vessel. A gangway, substantial straight ladder, or Jacob's ladder was not used for safe entry or exit of the vessel. The distance from the vessel to the white 5 foot stair case located on the floating dock measured to 24 inches in width and approximately 5 feet in height. This condition potentially exposed employee(s) to slip, trip, fall, and drowning hazard(s).
