29 CFR 1926.502(i)(4): All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard: On or about April 3, 2023: Employees engaged in scaffold erection activities at the Ashland Middle School construction site located at 806 North 17th Avenue, Ashland, Nebraska were exposed to fall hazards. The employer failed to ensure that covers for holes on the area C roof were color coded or marked with warning words.
