29 CFR 1910.119(e)(1): The process hazard analysis was not appropriate to the complexity of the process and did not identify, evaluate, and control the hazards involved in the process. On or about March 21, 2023, at Borden Dairy of Austin, TX, in accordance to the schedule set forth in paragraph (e)(1) of this section, the employer did not complete a compilation of written process safety information before conducting any process hazard analysis required by the standard. This violation was most recently observed in the anhydrous ammonia engine room when the employer failed to document and maintain accurate process safety information (PSI), such as but not limited to materials of construction for the following process equipment: a) HTR-1 to EV-1 piping segment; (PID BAUTX-09 rev5 to PID BAUTX-013 rev3); b) HPL to HTR-1 piping segment; (PID BAUTX-07 rev4 to PID BAUTX-09, rev5); c) HTR-1 to PH-1 piping segment; (PID BAUTX-09 rev5, to PID BAUTX-14, rev4). This exposed employees to inhalation, dermal burns and death.
