29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees. On or about December 15, 2022, and at times prior thereto, employees were working around an open floor hole on the third level of a new apartment building under construction without the hole being covered and secured, exposing employees to a fall hazard of over 6-feet to the concrete garage below.
