29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees: a) De-barker area: On or about December 7, 2022, and times prior to, employees were exposed to caught-in and struck-by hazards when the employer did not ensure that employees affixed a lockout device to each energy isolating device for equipment such as, but limited to, the de-barker and conveyors before entering the access-restricted areas where unguarded equipment was located.
