29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance: (a)Bell Incorporated at 609 West Algonquin Street, Sioux Falls, SD 57104: On or about and at times prior to November 29, 2022, the employer did not ensure rules and techniques involving specific energy control procedures to control hazardous energy were implemented which resulted in exposure to the hazard due to multiple energy source and pieces of equipment to include, but not limited to, the iPal Palletizer Machine Fanuc Robot R-2000i C 125L. This condition exposed employees to crushing, amputation, shock, and air embolisms. (b)Bell Incorporated at 609 West Algonquin Street, Sioux Falls, SD 57104: On or about and at times prior to November 30, 2022, the employer did not ensure rules and techniques involving specific energy control procedures to control hazardous energy were implemented which resulted in exposure due to multiple energy source pieces of equipment to include, but not limited to the Jagenberg Diana 105 gluing machine. This condition exposed employees to crushing, amputation, shock, and air embolisms while maintaining equipment. (c)Bell Incorporated at 609 West Algonquin Street, Sioux Falls, SD 57104: On or about and at times prior to November 30, 2022, the employer did not ensure rules and techniques involving specific energy control procedures to control hazardous energy were implemented which resulted in exposure due to multiple energy source pieces of equipment to include, but not limited to the Jagenberg Diana 98 HS gluing machine. This condition exposed employees to crushing, amputation, shock, and air embolisms while maintaining equipment. (d)Bell Incorporated at 609 West Algonquin Street, Sioux Falls, SD 57104: On or about and at times prior to November 30, 2022, the employer did not ensure rules and techniques involving specific energy control procedures to control hazardous energy were implemented which resulted in exposure due to multiple energy source pieces of equipment to include, but not limited to the Jagenberg Diana 90-2 gluing machine. This condition exposed employees to crushing, amputation, shock, and air embolisms while maintaining equipment. (e)Bell Incorporated at 609 West Algonquin Street, Sioux Falls, SD 57104: On or about and at times prior to November 30, 2022, the employer did not ensure rules and techniques involving specific energy control procedures to control hazardous energy were implemented which resulted in exposure due to multiple energy source pieces of equipment to include, but not limited to the Jagenberg Diana 90-1 gluing machine.
