29 CFR 1910.140(c)(12): The employer did not ensure that anchorages used to attach to personal fall protection equipment are independent of any anchorage used to suspend employees or platforms on which employees work. The employer did not ensure that anchorages used to attach to personal fall protection equipment on mobile work platforms on powered industrial trucks are attached to an overhead member of the platform, at a point located above and near the center of the platform: On or about 10/25/2022 at the jobsite located at 1 North Breakers Row, PALM BEACH, FL 33480, employees were exposed to a 67 foot 7 inches fall hazard due to the anchorage used for employees' personal fall arrest system not being independent from anchorage used to suspend employees' bosun chair.
