29 CFR 1910.333(b)(1): The employer did not ensure that exposed deenergized parts were locked out and tagged in accordance with paragraph (b) and treated them as energized parts. 1531 Grafton Road, Millbury, MA: On or about October 20, 2022, parts were not locked out or tagged in accordance with paragraph (b) of this section and appropriate PPE should have been worn when working on a disconnect switch near or on a 480-volt bus duct systemwhich may or may not have been energized.
