29 CFR 1910.28(b)(11)(ii):(ii) Each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as follows: (see TABLE D-2) Employees engaged in grain operations at 16515 County Road 14, Chappell, Nebraska 69129, were exposed to fall hazards in that employees were using a stairwell for access and egress to and from the office building and the stairwell system was not equipped with a stair rail system as defined by this standard on table D-2.
