29 CFR 1926.502(d)(15): The anchorage used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. Location: 109 Chase Street, Marlborough, MA; On or about 09/09/22, an employee was exposed to a fall hazard of 17-feet to the ground below when using a personal fall arrest system consisting of a Honeywell I294 temporary roof anchor that was improperly installed, in that, only a total of four (4) 16d nails (2 nail per metal flap) were used to secure the ring to the ridge beam. According to the manufacturer, the minimum number of fasteners required to achieve the 5,000 pound anchor strength is a total of twelve (12) 16d nails (6 nails down the center of each metal flap). Additionally, the anchor point was installed horizontally along the ridge beam in the manufacturers "flat roof" configuration, instead of the "pitched roof" configuration which would have had the anchorage point flaps straddling the roof peak on either side.
