29 CFR 1910.28(b)(3)(iv): Each employee is protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used. With respect to an establishment located at 1400 Plover Road, Plover, WI 54467, the employer did not ensure that employees were protected from fall hazards by a self-closing gate or offset: a) on or about September 27, 2022, employees were exposed to falls of approximately six feet, to the concrete surface below, when working near the entrance to the elevated platform above the slicers. There were two points of access to the maintenance platform above slicers that were not guarded. b) on or about September 30, 2022, employees were exposed to falls of approximately nine feet, to the concrete surface below, when working near the entrance to the maintenance platform for the Add Water tanks. c) on or about September 27, 2022, employees were exposed to falls of approximately five feet, to the concrete surface below, when working near the entrance of the maintenance pit in the Add Water area. d) on or about September 30, 2022, employees were exposed to falls of approximately eight feet, to the concrete surface below, when working near the entrance to the maintenance platform near canning line #12. e) on or about January 10, 2023, employees were exposed to falls of approximately nine feet, to the concrete surface below, when working near the entrance to the electrical maintenance platform near the end of palletizing line #1.
